Summary

The Home Care Liaison is a registered nurse who has a thorough understanding of available home care services and products, eligibility requirements, and develops the transition plan for safe return to home with the right home based services. The Home Care Liaison acts as a resource to Hospital/Sub-acute facility involved in home based care planning, and facilitates continuity of care for the patient referred to receive home based services. The Liaison meets with the patient/caregiver to determine needs and develops a safe transition for the home care team. The Liaison is actively involved in marketing and provides educational programs to promote the continuum of care needs for patients.

Qualifications

  • Graduate of accredited RN program required; BSN preferred
  • Thorough knowledge of home care (home health, infusion, and medical respiratory and equipment) eligibility requirements, agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services
  • Thorough knowledge of marketing and sales principles
